Foster Application

FOSTER PARENT APPLICATION

As a Foster for HSL, Magnolia Chapter, please understand that we do not have a building;
therefore, as a Foster you must be prepared to care for your Foster until a forever home is
found; no matter how long it takes. Some animals get adopted or picked for transport quickly
while others may take longer to get picked for transport or adopted.
 
The following is what is expected of each Foster Parent who agrees to take on an animal
through Magnolia Chapter. 
 Agree to a home check/visit by a Magnolia Chapter Official.
 It’s mandatory for all Foster Parents to attend at least one meeting a year.
 Provide socialization, love, shelter, food and care for the pet (Magnolia Chapter provides
all Vet care). 
 Keep accurate and complete records on care given to the animal. This information must
be recorded on an Intake Form which will be provided to you. The information includes
Vaccination Labels & Dates Given and records of all treatment (flea and heartworm
preventative, de-worming dates, spay/neuter date, etc). This record (Intake Form) is vital
for adoptions/transports and should be with you during adoption events.
 Understand that depending on the foster type, age and/or health other care may be
required. 
 Be willing to attend at least one Pet Smart adoption event per month once your Foster
animal is completely vetted, spayed/neutered (Pet Smart Requirement). Magnolia
Chapter meets at Pet Smart in Covington on Sunday (3 times per month) from 11a.m. to
3p.m., and we meet at Pet Smart in Slidell on the first Sunday of the month. This is the
routine schedule, unless otherwise announced on the Group Facebook Page. Please
bring your Foster to adoption events with a crate, leash, water bowl and most importantly
updated Intake Form and Vet Records (if any). If you do not have a crate, please let us
know in advance. 
 Provide pictures and updated information on your Foster animal. You may choose your
Foster’s name but please remember to be creative. We prefer unique names to avoid
confusion. (We have had many Peanuts, Lucky, Bella, Max, Lil Bit, etc.). Please check
your name choice with an officer before publishing.  
 Fenced yards are preferred for Magnolia Fosters. However, on a case by case basis, we
may accept leash walking in lieu of a fence. The Foster MUST have designated times to
go outside for bathroom outings, play and time out of the kennel.

Humane Society of Louisiana, Magnolia Chapter offers assistance in various ways for Foster
parents of rescued pets. Magnolia provides veterinary care for the Foster animal(s). The
procedures are as follows:  
 Vaccinations, de-wormer, monthly heart worm medication and monthly flea preventative
can be picked up on Sundays at Pet Smart Adoption Events from Beth Eaton. If for
some reason you cannot meet on a Sunday, please contact Beth Eaton to make
arrangements to meet you. All animals must be de-wormed immediately when coming
into the Foster program.
 All animals 3 months and older must be on a monthly flea & heart worm prevention
(regardless if tested positive or negative).
 All dogs over 6 months old must be tested for heart worms. This is done with our Vet.
 All cats must be tested for feline leukemia and be negative. 
 All puppies/kittens 9 months or younger require 4 vaccinations no longer than 3 weeks
apart.
 All dogs/cats over 9 months to 11 months require 3 vaccinations no longer than 3 weeks
apart.
 All dogs/cats 12 months and older require 1-2 vaccinations no longer than 3 weeks
apart.
 For Foster animals picked for transport, it is imperative for Foster Parents to follow
protocol told to you for worming and so forth. Foster Parents must be able to get their
animal(s) to transport on designated day with completed paperwork.
NOTE: It is Foster Parent’s responsibility to keep track of the dates when their foster animal(s)
need their monthly medications and upcoming vaccinations. If vaccinations lapse (exceed 3
weeks) the entire vaccination process has to start over. This creates additional expenses for
Magnolia Chapter and delays adoption and/or transport.
 All emergency medical issues require transport to our Veterinarian in Gulfport, MS by the
Foster Parent or a Magnolia Chapter volunteer. Please contact an Officer during this
situation.  
 Spaying/Neutering of your foster animal(s) must be done by 4 months of age. If you
foster an adult animal then it must be done as soon as possible. This can be done two
ways:
1. An appointment can be scheduled at our Vet in Gulfport, MS. You should expect to
drop them off at approximately 11:00 a.m. and pick up the same date at
approximately 4:00 p.m.

OR
2. You can bring your Foster to Magnolia’s Spay/Neuter Clinic just outside of Bogalusa.
You will need to call the appointment line 985-241-4290 and follow prompts. Be sure
to identify yourself and animal as a Magnolia Foster needing to schedule an
appointment. A volunteer will call you back the week of the clinic. Clinics are held
twice per month. PLEASE make sure you have voice mail set up on your phone so a
message can be left if you’re unable to answer. Be prepared to have your Foster at
the Clinic at 8:00 a.m. on your appointment date and pick up later that same evening
when called. Foster Parents are responsible for post-op care. You will be given a
care sheet and the number to our Vet.

ADOPTION INFORMATION
 If anyone is interested in adopting one of the animals you are fostering, they must
complete an application at magnoliahumane.rescuegroups.org and the application must
be approved by an officer. (NOTE: Just because someone submits an application does
not mean they will be approved to adopt the animal they are applying for). You will need
to prepare to have your Foster pet have a "meet and greet" with the approved potential
adopter. "Meet and greets" are required before adoptions. Adoption fee is $85.
 For Dogs: Please understand that when adopting out any large breed dog the potential
owner must have a fenced yard; no electric fences allowed. Small/medium sized dogs
fence requirements are a case by case basis. 
 For Cats: All cats are indoor cats only and de-clawing is not allowed. 
 Any adoption must be done with an officer or a senior volunteer. Intake forms must be
completely filled out and all other paperwork received from Spay/Neuter clinic must be
present for adoptions to take place. All animals must be in good health and have good
temperament to be adopted out or to come to adoption events. Any foster animal that is
returned after an adoption will be returned to the foster caretaker who had them.
 If you wish to take in an animal that an owner wants to surrender, the pet must be
spayed or neutered and up to date with vaccinations, as well as treated for internal and
external parasites before Magnolia will be able to offer assistance. The type of
assistance we will be able to offer will vary based on available resources at the time. We
do understand that there are some instances where having all of the above is not
possible given the owners conditions so we will consider that. 
 Puppies pulled from a shelter must be held in Foster for 10 days before the puppy can
be offered for adoption.  Adopter must agree to Spay/Neuter by the age of 4 months and
must provide proof of completion to Magnolia.
All Foster Parents must read, agree to and sign this document.
 
I am applying to become a Foster Parent with the HSL, Magnolia Chapter. I have read and
